Long Term Care Diversion Plan

The first  goal of the American Eldercare Plan is to:

• keep you in your  home setting

• provide quality home health care

• provide access to community based services 

• delay or avoid placement in a nursing facility

• coordinate acute and long term health care needs

   We offer care to meet your individual needs.

Type of services that may be provided

• Care Management - evaluation, planning and coordination of services

• Home Health Care - bath visits, homemaking, companion and respite care

• Adult Day Care - caregiver support and respite care in an active, socialized setting

• Assisted Living Facility - providing 24 hour supervision, assistance, socialized peer interaction, dining and housekeeping services

• Prescriptions - no co-payments for covered drugs prescribed by a physician

• Consumable Medical Supplies - incontinent supplies and nutritional supplements

• Home Delivered Meals  - nutritious meals delivered to the home

• Emergency Response System - providing 24 hour access to emergency services

• Eye Glasses - one pair every six months as needed

• Hearing Aides - one every three years

• Co-Insurance - Medicare deductible and co-payments are paid per the Medicaid rate

What is the cost?

The Long Term Care program is FREE for anyone that meets the Florida State eligibility requirements. 

Are you eligible?

Eligibility is determined by the Department of Elder Affairs Cares Unit and the Department of Children and Families.  These are the agencies that  decide who gets enrolled in the plan.  American Eldercare takes care of all the services after that.  The eligibility requirements include:

• Age 65 years of older

• Have Medicare parts A & B, or a Medicare HMO

• Reside in a participating Florida county

• Meet Florida Medicaid financial requirements

• Meet physical requirements as determined by the State

• Must NOT be currently enrolled in a Hospice Program
